I understand that the access is subject to revocation if incomplete/incorrect information is given or if published rules are violated. I understand that I am responsible for the access card issued under this form as long as it is in my possession and that a replacement fee of AED 265 for DMCC card and AED 100 for other cards will be charged for replacement if card is lost, duplicated or has become defective. Alteration, Duplication, Misuse, Transfer or Resale of this access card is prohibited.
